Important Information for Patients
These are undiscounted gross charges required to be posted by Alaska law. The amount you actually pay may be significantly lower depending on your insurance benefits, negotiated rates, financial assistance, or self-pay discounts.
Upon request (and required for all uninsured/self-pay patients), we will provide you a written Good Faith Estimate of anticipated charges before non-emergency services are rendered.
AK Chiropractic is a participating, in-network provider with Premera Blue Cross Blue Shield of Alaska and several other plans. Insured patients will only be responsible for applicable co-pays, coinsurance, and deductibles.
No-Show / Late Cancellation (less than 24 hour notice) Fee for massage: $50.00
No-Show for New Patient Exams (after first occurrence): $150.00 (this is a scheduling fee, not a health care service).
